Lead-Based Paint Application Forms

Note: Within any calendar year, cumulative course accreditation fees shall not exceed $5,000 for an individual training provider.

Lead-Based Paint Questionnaire and Exemption Form and Abatement Permit/Notification Forms*
The exemption form is provided in an effort to expedite the permit process. The questionnaire and exemption form must be completed and submitted on all projects involving buildings and structures built prior to 1978. Projects determined to be exempt need not be submitted to the Department of Health for lead permits. For projects requiring lead permits use the permitting and notification application forms.

Fees for Lead-Based Paint Hazard Reduction and Control Permits are $40 plus 3% of the abatement cost.
