License fees for ambulatory surgical treatment centers are determined by the number of surgical procedures the center performed in the previous fiscal year. Enter the appropriate license fee from the table below in the
ambulatory surgical treatment center
application. The annual license fee is due 90 days before
expiration of your current license. A late fee is charged if payment is received after that date.
