How to File a Health Facility Complaint

Patients/Residents/Clients/Family Members/consumers and providers of health care services, delivered in licensed and/or certified hospitals, nursing homes, group homes for the mentally retarded, home health agencies, dialysis centers, ambulatory surgical centers, hospice care agencies, and medical laboratories, have a responsibility to promote the highest quality of care. If you witness a suspected violation of health department regulations or patients' and residents' rights (physical or mental abuse, neglect misappropriation of property, or unexplained injury) call the Health Care Facility Division's Complaint Desk (HCFD).

Prompt action is vital. If you see something that may be a violation, jot down all you know about it (location, date, time, names, and other pertinent information). You can reach the HCFD Complaint Desk during working hours (8:30 am - 4:30 pm) at (202) 442-5833. At other times a voice mail message may be left including your name and telephone number. A staff member will contact you during the next workday regarding your hot line message.

Complaints can be anonymous, but it helps HCFD to know who you are, your name, your address, and telephone number if more information is needed and to let you know the results of the investigation.
