Notice of Public Hearing

Pursuant to the Health Services Planning Program Re-establishment Act of 1996, DC Official Code § 44-406 (b)(4), the District of Columbia State Health Planning and Development Agency ("SHPDA") will conduct an information hearing on Certificate of Need Registration No. 04-2-6, an application by 1330 OpCo, LLC to acquire the assets of Thomas House, a 53-bed nursing home at 1330 Massachusetts Avenue, NW. The hearing will be held on Friday, January 28, 2005 at 10:00 am, at 825 North Capitol Street, NE, 4th Floor, Room 4131, Washington, DC 20002.

Persons who wish to testify should contact the Project Review Branch of the SHPDA on (202) 442-5875 before 4:45 pm, Thursday, January 27, 2005. Each member of the public who wishes to testify will be allowed a maximum of five (5) minutes. Written statements may be submitted to:
