DC Healthy Families
Application
You should apply for the DC Healthy Families Program if you are:
  • A parent
  • A child under age 19
  • A pregnant woman

To get an application:

  • Print* and fill out the Application Form (English Version)
  • Complete* la siguiente aplicación, luego imprimela (Spanish Version) span
  • Call 1 (888) 557-1116 to obtain the French or Vietnamese version of the DC Healthy Families application
  • Call 1 (888) 557-1116 for general information
  • If hearing-impaired, call the TDD/TTY number 1 (877) 6PARENT
  • Pick up an application at your local library, Giant, Safeway, CVS, and Rite Aid

Medical services are provided through a network of doctors located throughout the District of Columbia. Once approved for DC Healthy Families you will get information in the mail explaining how to take the next step.
