DC Diabetes Prevention and Control Program Patient Information and Resources Community Outreach
The Department of Health, Diabetes Prevention and Control Program (DPCP) works with community
organizations to provide wellness classes, support groups and educational forums.
These programs create a structure for people with diabetes to manage their condition outside
of traditional health care settings.
Overall, understanding proper diabetes management is an important factor in controlling
diabetes. The DPCP encourages patients and physicians to work together in
controlling diabetes. It is important for people with diabetes and their physicians to
monitor their HbA1Cs, (a three month average of blood sugar levels), blood pressure and cholesterol.
Exercise and proper nutrition is key to living a healthy life with diabetes. Patients should make it a point ask their diabetes
health care professional about any questions and concerns they may have. Communicating with
a diabetes health care team is important to good diabetes control.
