DC Diabetes Prevention and Control Program Special Programs
Advisory Council Subcommittees Surveillance/Data
Reviews current incidence and prevalence rates for diabetes, the disease's serious complications;
rates of disability and premature mortality; determines gaps.
Makes recommendations for improving the quality of data collection/surveillance included in the
Diabetes Prevention and Control Program (DPCP's) five-year plan for control of diabetes.
Standards of Care
Reviews diabetes mellitus standards of care developed by the American Diabetes Association, various states and the Health Plan Employer Data and Information Set (HEDIS).
Makes recommendations of standards of care for the District of Columbia; the recommendations will contribute to the development of a five-year plan for the control of diabetes and its complications.
Community Intervention/Provider Education
Assesses diabetes complications and barriers to glucose control.
Recommends 1-2 interventions. Provides insight into increasing adoption of the team approach by primary care providers and managed care organizations (MCOs) in diabetes care.
Makes recommendations for community interventions and provider education to be included in the five-year plan for control of diabetes and its complications.
